cystic acne is a condition of the skin related to a viral infection. What your boyfriend has is likely not cystic acne, but a plain ol’ cyst.
Cystic acne is when a small nodule of infectious pus developes far under the out layer of skin (normal acne developes on the skins surface and is usually controlled by over the counter acne medications.) cystic acne is extremely painful when touched, and often times the pressure of the nodule will cause headaches or painful swelling. they can be tiny or as large as the tip of your finger, and they come and go usually on a monthly basis. They can be controlled or gotten rid of by taking a drug called "acutane" or some such similar compound available only by prescription, but I believe there are currently studies being done to develope a newer drug.

    Surgical removal.

Although a doctor would probably not recommend it, lots of sun exposure will remove acne. Don’t burn yourself. You could also try tanning salons.

I have acne and I don’t think that’s the problem. Stick a sterilized needle in it and let it drain. Then clean it with alcohol.

If it shows pus, assume that it is acne.
Otherwise it can be a skin growth or keloid.
keloid are big skin growths that are scar tissues.
Did your b/f have any small injury at this place?
Healing of that will form the keloid.
